It is essential that we gain a more comprehensive understanding of therapeutic approaches in EP.

To minimize vascular extravasation, IVUS-guided HDR and EP interventions are preferred. Although retrograde approaches are now routine for most operators, a major challenge arises when 3G GW fail to advance the lesion. Simply increasing tip load and relying only on the wire greatly raises the risk of perforation. At that point, the ability to apply adjunctive techniques such as KWT or Carlino becomes crucial. Yet, many Japanese practitioners still lack sufficient proficiency in these advanced methods.
