世界標準のCTOの治療。

 CTO2024をTCTMDから見てみた。Hybrid approachである。標準化した治療でCTOを治療しようというコンセプトである。

簡単にいうと短いCTOであれば、ワイヤーでドリリングはいいが、２Cmこえたら、ナックルで奥までいって、レトロからナックルで治療。もしくは、奥でstingryという方法です。で、基本はガイドライナーをいれて、RCARTを素早くやるという方向になっています。

これだと、早い。あとは、６Frで治療が可能である。

日本式のワイヤーで進んでいく時の欠点。

１、遅い。２、穴があく、３穴を開けるとナックルが安全にできなくなる。

なので、最初から、PJWを使うわけです。その基本がBASEになるわけです。BASEがしにくいもしくはできない状況、しか硬いワイヤーが使いません。

欧米式のワイヤー利点。

１早い、２穴が空きにくい、３、誰でもできる。

欧米式の欠点は、手技の見た目が美しくない。

日本式の時間を気にしない硬いワイヤーによるCTOの以前の治療は、世界からみたら、リスペクトではなく、容認できない古い治療と認識されているような気がします。

そこに気がつかないといけないと思います。

In 2024, there is a notable shift in the approach to Chronic Total Occlusion (CTO) treatment, as observed from TCTMD. The emphasis is on a hybrid approach, aiming to standardize CTO interventions.

In brief, for short CTOs, using wires for drilling is considered effective. However, for CTOs exceeding 2 cm, the approach involves reaching deep using a knuckle or employing the retrograde technique with a knuckle or the stingray method. The fundamental concept revolves around inserting guide liner quickly performing RCART .

This approach is characterized by its speed, with the possibility of using a 6 French (Fr) catheter for treatment. Notably, the traditional Japanese method of advancing with wires is criticized for being slow and prone to complications such as perforations that may hinder safe knuckling.

Consequently, the preference is shifting towards using  PJW from the outset, forming BASE. If BASE is challenging or impossible, rigid wires are  utilized.

Advantages of the Western approach with wires include speed, reduced risk of perforations, and accessibility for practitioners of varying skill levels. However, a notable drawback is that the procedural aesthetics might not be as visually appealing.

The previous treatment paradigm in Japan, relying on time-consuming procedures with rigid wires for CTO, seems to be perceived globally as outdated and not worthy of respect but rather as an antiquated form of treatment that needs acknowledgment. Recognition of this shift is essential, as it signifies a departure from the conventional Japanese approach to CTO treatment and an acceptance of the more contemporary, standardized methods prevalent in Western practices.
