RCA CTO ２年前にinvestmentで終了している症例。
８frで＃２までチャンネルがあり、その後にmicrochannelでPDがでてきた。
その後に、＃4AVにXTA UB３で末梢に通過。その後にIVUSで石灰化の部分でTDADR通過しなおして、＃４AVを全部治療。２．５DCBで終了。＃４PD小さくが、４G超えていたので、今回は、ここで終了したが、造影はとてもきれであった。
石灰化のところにSTをreshapeして穿刺できた。症例
４PDはその穿刺したところの手前にIVUSでみえた。
It started at 8frGC. There was a channel to #2, then a PD in microchannel.
Then XTA UB3 to #4AV passed distally. Then IVUS with ST in the calcification area to re-pass TDADR and treat large #4AV. 2.5 DCB and done. Due to the small #4PD, this time, it was left in place because it was flowing.
The ST could be reshaped and punctured at the calcification.
4PD was seen on IVUS in front of the puncture site.
