SFACTOクロッサー抜き

短い病変でしたが、クロッサー単独でCTOをぬけました。CTOのためのデバイスとされて、CTOに使用していますが、皮一枚ぬけないことが、多く今回はたまたまぬけたのでしょうか？CTOをクロッサーが通過するときは、動画のようにものすごいスピードでおせます。ただし、経験的にIVUSでたしかめると、血管内のへりちかくを走行していきます。我々の治療するSFAのCTOの35ナックルとの大きな違いは、ｓFAがCTO断端まで到達する時間短い、SUBのときの穴が小さいので、そこから１４GWで抜けやすい。クロッサーを使うようになってから、アンテのみの成功率があがっています。
35ナックルのsubは、subの内腔が大きく、IVUSガイドでワイヤーをtrueに抜こうとしても、ワイヤーがなかなかひっかからず、苦労しますが、クロッサーのsub穴は小さくどこからでも、ワイヤーを操作できる感覚です。あとは、皮一枚のときは、9/40による袈裟斬りも可能です。

クロッサーの方向は、ワイヤーnavede５０を先端をださないように仕込んで、方向をかえています。

SFACTO Crosser Penetration

This CTO was short, it was able to pass CTO with Crosser singularity.   Crosser is designed for CTO device and I use as it is for, though it doesn’t  ofen cross CTO lsesion easily.  Well, it passed by chance  in this time?  Usually, when Crosser penetrates CTO, it passes in high speed like attached moving image up.  However, it seems Crosser runs near intimal lip at IVUS image.  The major difference with 35 knuckle of the CTO in SFA is that the short time when SFA reaches at the CTO lip and Crosser make smaller subintimalspace than 35knucke. So 14 guide wire penetrates CTO easily  than 35 knuckel through the small space of subintimal.  The success rate rises only for AnteGrade since we come to use Crosser.

There is difficulty to pass the GW using IVUS guide through true lumen because subintimal of 35 knuckle has large lumen so it is hard to find place the wire.  You may feel freedom to operate wire in direction as you like since subintimal hole of Crosser is small.  At Intimal, Confienza 9-40 with tip deflection is the one of the options to cut sharply as Japanese SAMURAI sword. 

Crosser is stocked in on wire Naveed 50 not to start the tip to change the Crosser direction.
